C-U Public Health District is partnering with Carle Hospital, OSF Healthcare, United Way of Champaign County, and the Champaign County Mental Health Board to conduct a Community Needs Assessment, in an effort to make Champaign County safer and healthier.
You can help by filling out a survey about our community’s health needs. It takes about 12 minutes and will not require you to divulge personal information.
The survey is available in English or Spanish.
